Thinking Ahead – Planning for the Future, Your Way

A new group project at Lonsdale Medical Centre

Have you ever thought about what matters most to you as you get older or approach the end of life? Where would you prefer to be cared for? What sort of treatment would you want, or not want? And how can you make things easier for your family and friends?

These aren’t always easy conversations to have, but thinking ahead can help ensure that your wishes are understood and respected.

We’re pleased to introduce Thinking Ahead, a new project offering patients a friendly, supportive space to explore these questions and learn more about planning for future care.

Led by a trained facilitator, the project involves three small-group sessions, each lasting 90 minutes. There will be opportunities to ask questions, share thoughts and learn about the choices available to you. You don’t need to have made any decisions beforehand.

What will we cover?

Session 1 – Why plan ahead?
Understanding Advance Care Planning, what you can plan for, when to start and how to go about it.

Session 2 – Understanding your options
Exploring different types of care plans, including the Universal Care Plan (UCP), Advance Decisions to Refuse Treatment and Do Not Attempt Resuscitation (DNACPR) decisions.

Session 3 – Putting your wishes into practice
Learning how to start your own Universal Care Plan and communicate what matters most to you.

When?
Three Thursday afternoons, starting 22 October 2026 at 2pm. Each session lasts 1½ hours.
