Patient-Led Chronic Illness Support Group

We are looking for interest in setting up a patient-led “chronic illness support group” at the practice: an informal, peer-led, community-based support for individuals facing the challenges of chronic illness.  If you are interested in either helping set this up or finding out more and getting involved please contact us: at admin.lonsdale@nhs.net with “Chronic Illness Support Group” in the title and for the attention of the practice manager: Michelle Reilly.  We look forward to hearing from you!
